We suggest that you fill in the above Order Form.  There is no Credit Card information taken.  If you wish to E-Mail your order, we suggest that you select and copy the Order Form into your E-Mail.  Most browsers / mail facilities will handle this.  If you wish to FAX or mail your order to us, you should print the Order Form using your browser's print facility.  

Prior to E-mailing, faxing or mailing your order, please make sure that the quantity ordered is correct and that your payment method is indicated.  Payments for Offline Orders are by Credit Card, Money Order or Personal Check.  Personal Check orders are held (10) business days for check clearance.
